By submitting this form you give your consent to Evobeds s.r.o., with registered office at Paličkova 75/2, Ostrava, 709 00, ID No.: 07754523 (hereinafter referred to as the "Administrator"), to use the following information in accordance with Act No. 101/2000 Coll, on the protection of personal data (hereinafter referred to as the "Personal Data Protection Act") and Regulation (EU) 2016/679 of the European Parliament and of the Council of 27. April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (hereinafter referred to as the "GDPR") to process such personal data:

    • first and last name
    • company name
    • e- mail
    • phone number

Name, last name, company name, telephone number, billing address and e-mail address are processed by the Controller for the purpose of providing services, making payments, issuing billing documents, registering domain names, conducting customer contests and sending sales and marketing communications. This data will be processed by the Administrator for a period of 3 years from the last order.

  2. You give your express consent to the above processing. Consent may be withdrawn at any time, for example by sending an email or letter to the contact details of the Controller.

  3. Please note that under the Data Protection Act you have the right to:

    • withdraw your consent at any time,
    • request information from us about what personal data we process about you,
    • request an explanation from us about the processing of your personal data,
    • request access to this data and have it updated or corrected,
    • request us to delete this personal data,
    • contact us or the Office for Personal Data Protection in case of doubts about compliance with the obligations related to the processing of personal data.
